COMPLAINT FORM

In an effort to support uniformity in insurance regulations, address industry concerns regarding the protection of consumers, and enforce a code of conduct for CLTC designees, the Board of Standards has instituted disciplinary proceedures. Please refer to section IV. The Disciplinary Procedures in the CLTC Code of Professional Responsibility for detail information about the disciplinary process. The Board will initiate a review of allegations against a CLTC designee once the following Complaint Form has been completed and filed with the Board.

For the protection of all parties, the Board will not conduct a review into any allegations unless all information on the complaint form is completed.
